Health priorities Worksheet

What does health mean to you, and how can you prioritize it? Discover your health priorities with the questions below!

INSTRUCTIONS:

  • Grab a pen and paper to answer these questions honestly. Write down your thoughts, no matter how big or small they might seem.

  • Aim for at least 3 strengths or responses to each question.

  • If your answer feels unclear, ask yourself: "Why is this important to me?", "What changes would improve my overall well-being?"

  • If your answer is not a strength ask, "What strength could lie underneath this?" or "What qualities helped me achieve this?"

  • Wrap up by identifying recurring themes or areas needing attention.

  • Finally, list your Top 5 Health Priorities on a separate piece of paper and create a plan to address them. Feel free to add one or two health priorities which are important to you, even if they're not in your top 5.
